Below is a list of the narrative questions. It is suggested you work off of these questions, so your information can be cut and paste into the new secure site.
1. How did your goals, mission, and programming stated in the application, serve or reach the community of Salt Lake County?
2. Have your goals, mission or programming changed from previous years? Please note any problems or changes from your application?
3. Describe what was accomplished using your ZAP funding and how ZAP funds assisted with your organization’s goals? If applicable, include a list of free days or events, and how they were publicly announced.
4. How did you measure and evaluate the success of your organizational efforts? What were the results of these measurements and evaluations?
5. Describe any noteworthy achievements and awards that occurred during this cycle using ZAP funds.
6. Describe the internal and external challenges and opportunities you faced during the past year, and how your organization addressed them?
7. What, if any, community issues (problems, challenges, opportunities) have you observed that impact your organization or the wider cultural community, and its audience/constituency? Do you have suggestions for addressing these issues? How could the ZAP program assist?
8. ZAP Tier I and Zoological funding was initiated to build organizational capacity and to create stability. How is your organization striving to achieve stability and sustainability? Is your programming or organization downsizing, maintaining, or growing in size? Please explain in detail how this decision was determined in relation to your long term stabilization and sustainability.
9. If all Zoo, Arts & Parks funds have not been expended, please attach an explanation detailing how and when the remainder of funds will be used.
